VETERINARY LICENSING REQUIREMENTS FOR THE STATE OF INDIANA

The following lists the general requirements for veterinary licensure in Indiana. For specific questions and a licensure/examination application, you must contact:

License by Endorsement:

  • Completed and signed application from the Indiana Board of Veterinary Medical Examiners
  • Affidavit for any positive responses to questions on the application
  • Fee of $150 and  One photograph - dated and signed across the back “I certify that this is a true photo of me.”
  • Official transcripts - approved program/certifying degree granted ECFVG Certificate - Notarized copy (For foreign graduates only - copy or original transcripts not required)
  • Verification of licensure form for all states where the veterinarian holds or has held a license
  • Verification of five (5) years active practice immediately preceding the application. In the form of a letter from employer, dated Yellow Pages listing, letters from colleagues, other documentation as determined by the Board Board approval, and
  • Law examination (passing score is 75).


License by Examination:

  • Completed and signed application from the Indiana Board of Veterinary Medical Examiners
  • Affidavit for any positive responses to questions on the application
  • Fee of $150 plus the cost of NAVLE
  • One photograph - dated and signed across the back “I certify that this is a true photo of me.”
  • Official transcripts - approved program/certifying degree granted
  • Certification of expected graduation date ECFVG Certificate - Notarized copy (For foreign graduates only - copy or original transcripts not required) PES Scores - if applying by examination and not sitting for the examination in the State of Indiana Verification of licensure form for all states where the veterinarian holds or has held a license
  • Verification of five (5) years active practice immediately preceding the application. In the form of: letter from employer, dated Yellow Pages listing, letters from colleagues, other documentation as determined by the Board Board approval, and 
  • Law examination (passing score is 75).
